1. Payment Due - The date payment is due. If payment in full is not received by this date, a late fee will be assessed on the unpaid balance
2. Prior Balance - Total amount due on the
previous balance.
3. Amount Due - Amount to be paid by the payment
due date. Includes prior balance, current charges, payments
and credits applied to the account.
4. Pager Phone Number - Pager number or personal
identification number.
5. Sale - Charge for paging equipment purchased.
6. Serial Number - Uniquely identifies your
paging equipment.
7. Paging Service Purchased - Charge for the
monthly paging service. A prorated charge for a partial period
of service may appear on your invoice. The partial month charge
on this invoice is for service from 12/15/04 to 12/31/04.
8. Enhanced Services - Charge for optional
enhanced service(s), such as Voicemail, Pager Replacement Program,
Custom Greeting, PageSaverSM, etc.
9. Overcalls - If you exceed the monthly call
limit, an additional per page fee is charged. You can not carry
over unused calls to the following month.
10. Contribution Reimbursements - We assesses a Federal Universal Service Charge to recover our costs of complying with certain governmentally imposed requirements.
11. Government Taxes and Fees - Includes government
mandated local, state and federal taxes, PUC surcharges and
federal and excise taxes where applicable.
